Referral Strategy Aims:

  • To ensure that long term support for people with aphasia is accessible, by:
    • ensuring that all health care, wellbeing and social services who support people with aphasia are informed about Say Aphasia's service.
    • ensuring Say Aphasia's service is part of the care pathway / integrated care systems for people with aphasia.

  2. Stroke and brain injury units and rehabilitation centres

  3. Acute and Community NHS SLT's

  4. Stroke Association

    1. Local Support Co‑ordinator

    2. Local Engagement Officer

  5. Headway

    1. CEO

    2. Local groups

  6. Age UK

    1. CEO

    2. Local groups

  7. Wellbeing and social prescribers

  8. Adult social care services

  9. Voluntary, Community and Social Enterprise (VCSE)

  10. VODG (Voluntary Organisations Disability Group)  

  11. Integrated Care Boards / Primary Care Networks.

    

Strategy Priorities:

  1. Email local organisations and head of departments:

    1. with info about our service + digital flyer

    2. ask for an online meeting to:

      1. Introduce ourselves and the charity in more detail.

      2. Discuss how we can work together to provide on-going support for people with aphasia

      3. Document the agreed plan of action. 

  2. Invite the team to a biannual meeting to ensure progress continues.
  3. Send out charity email to all referrers, biannually, with the aim of:

    1. Being in the forefront of their minds.

    2.  Offering insight into our service outcomes and achievements on how we are improving the lives of those with aphasia.
